For TGV and other high-speed trains, you can usually book way in advance, like 3-4 months out. That's usually when you get the best prices too.

It should be on the final payment step, before you actually confirm everything. Look for a little box that says 'promo code' or 'discount code'.

It really depends on the ticket type. Some are more flexible than others. I cancelled a regular point-to-point ticket once and got most of it back, but there was a fee. Passes can be a bit trickier.

Mine was an e-ticket last year, they emailed it to me. Just printed it out as a backup, but mostly used my phone. Pretty sure some things are still paper though, might want to double check for your specific pass.
